A Smooth Transition…
…is our goal at Comfort Home Health Care. Whether the
client is coming home after a hospital or nursing home stay, or just
looking forward to a bit of assistance around the house; the nurses at
Comfort Home Health Care work very hard to make sure everything is
taken care of.
Referral & Assessment When
Comfort Home Health Care receives an initial referral call, the intake
nurse will begin collecting preliminary medical information and
expected staffing needs. The nurse will then schedule an in-home visit
with the potential client and family.
Developing a Care Plan
Once in the home, a nurse will obtain a complete medical history and
perform a physical assessment. The nurse will then work with the
client, the family, physician and other healthcare and other concerned
parties to develop a personalized care plan. At Comfort Home Health
Care, we are dedicated to ensuring that the client remains involved in
directing his or her care.
Ongoing Care Coordination
After the formal intake is completed, the nurse's duties shift to
ongoing case management. The nurse supervises caregivers, coordinates
services with outside healthcare providers, consults with the physician
and, most importantly; maintains ongoing communication with the client
and family.
Discharge Planning We measure our
success in our ability to help clients and families regain their
independence. When the client's goals are met, Comfort Home Health Care
works to ensure a smooth transition from our care. The nurse will tie
up any loose ends such as arranging supplies or coordinating ongoing
community resources. Should any future needs arise, Comfort Home Health
Care is just a telephone call away.
