The staff at Maple Manor Health Care & Rehabilitation recognizes
that the transition of going from a home or assisted lived setting
into a nursing home can be a difficult one, both financially and
emotionally. The Social Services Department at Maple Manor Health
Care & Rehabilitation is dedicated to making this transition as
smooth as possible for the resident and family. We are here to
answer your questions and walk you through the admission process.
Many of our residents are admitted to us directly from hospitals,
for both short and long term stays. We also accept admissions
directly from home. In order for a resident to be admitted directly
from home, a Pre-Admission Screening must be performed, prior to
admission, by Olmsted County no sooner than ten days prior to the
date of admission. A history and physical form also needs to be
completed, prior to admission, by the resident's physician no sooner
than five days prior to the date of admission. Our business office
is available to answer questions about insurance and the Medicare
and Medicaid programs available to assist residents. |
